Most of what you’ll read about keto and inflammation falls into one of two camps: breathless claims that it “cures” autoimmune disease, or dismissive warnings that all that saturated fat will inflame your arteries into oblivion. Both miss the actual picture, which is messier and more conditional than either side admits.
Here’s what the research actually shows, and what I’ve seen work (and not work) in practice.
Why Keto Might Reduce Inflammation in the First Place
The leading explanation involves beta-hydroxybutyrate, or BHB, the main ketone your body produces when carbohydrate intake drops low enough. A 2015 study in Nature Medicine found that BHB directly inhibits the NLRP3 inflammasome, a molecular complex involved in triggering inflammatory responses linked to conditions like gout, type 2 diabetes, and Alzheimer’s disease. That’s not marketing. That’s a specific mechanism with decent supporting evidence.
Beyond BHB, cutting refined carbohydrates and sugar drops blood glucose and insulin levels, and chronically high insulin is itself pro-inflammatory. So there’s a dual effect: the ketones do something useful, and the stuff you stopped eating was causing harm.
Then there’s your gut microbiome. Reducing fermentable carbohydrates (the kind that feed certain bacterial populations) can shift inflammatory signaling from the gut. The research here is genuinely mixed, and some of those bacterial populations you’re starving out were doing beneficial things too. Worth knowing.
Where the Evidence Is Strongest
Epilepsy. Ketogenic diets have been used to reduce seizures since the 1920s, and the anti-inflammatory and neuroprotective effects are part of why they work. That’s the most established ground.
Beyond that, the clearest clinical signal in recent years has been in metabolic conditions. A 2020 study in Frontiers in Nutrition showed significant reductions in CRP (C-reactive protein, a standard inflammation marker) in people following a ketogenic diet for 12 weeks or more. But here’s the catch: the participants who showed the biggest improvements were also losing weight, which complicates interpretation. Fat loss independently reduces inflammation. Separating “keto did this” from “losing 20 pounds did this” is harder than most articles acknowledge.
Rheumatoid arthritis and other autoimmune conditions are where you’ll find the most anecdotal enthusiasm and the thinnest clinical evidence. Some people report dramatic improvement. Some see no change. The honest position is that the data doesn’t yet support recommending keto specifically for autoimmune disease, but it absolutely doesn’t rule it out. If you have an inflammatory condition and want to try it, that’s reasonable, with your doctor involved.

This is where I part ways with most keto coverage.
A poorly designed ketogenic diet, meaning the kind a lot of people actually eat, can be pro-inflammatory. If your fat sources are mostly processed meats, cheap industrial seed oils, and dairy from grain-fed animals, you’re loading up on omega-6 fatty acids and potential inflammatory compounds while skimping on the anti-inflammatory foods that actually make keto work.
The omega-6 to omega-3 ratio matters. A lot. The typical American diet runs somewhere around 15:1 or 20:1 (omega-6 to omega-3). That imbalance correlates with higher inflammatory markers. A keto diet built on bacon, processed cheese, and pork rinds doesn’t automatically fix that ratio. A keto diet built around fatty fish, olive oil, avocado, leafy greens, and grass-fed meat gets you much closer to the 4:1 ratio that research suggests is beneficial.
Practically speaking, these are the swaps that make the difference:
- Sardines, salmon, and mackerel over deli meat as your go-to protein
- Extra-virgin olive oil (something like California Olive Ranch, widely available and reasonably priced) over vegetable or canola oil for most cooking
- Grass-fed butter or ghee over standard grain-fed dairy when possible
- Actual green vegetables (spinach, arugula, broccoli, zucchini) making up a meaningful portion of your plate, not just a garnish
The magnesium and zinc in those vegetables matter too. Both regulate inflammatory pathways, and both tend to be low in people eating ultra-processed food regardless of macros.
Measuring Whether It’s Actually Working
If you’re using keto specifically to manage inflammation, track something measurable. A three-month experiment based on how you feel is a start, but subjective improvement is easy to confuse with placebo, seasonal variation, or the general benefits of cleaning up your diet.
Ask your doctor to run a basic inflammatory panel before you start: CRP, ideally high-sensitivity CRP (hs-CRP), and if they’ll order it, IL-6. Get it repeated at 12 weeks. That’s enough time for meaningful change to show up if it’s going to.
Joint pain, skin conditions like psoriasis, and energy levels are reasonable secondary signals, but I’d treat them as supporting evidence rather than proof. I’ve had clients convince themselves keto “fixed” their inflammation based on feeling better in the first two weeks, which is almost entirely explained by water weight, reduced bloating, and the novelty effect of any new dietary change. Give it real time and use real markers.

I want to be direct about something. For general weight loss, food quality on keto is somewhat flexible. People lose weight eating Quest bars and processed cheese because the caloric and hormonal mechanisms still function.
For inflammation, food quality is not flexible. The anti-inflammatory effect depends significantly on what you’re eating, not just what you’re not eating. A 2019 analysis in Nutrients looking at Mediterranean-style low-carb diets (which emphasize whole foods, olive oil, and fish) showed better inflammatory outcomes than standard low-carb diets higher in processed meat and dairy.
If inflammation is your goal and you’re not seeing results, look at your fat sources before you look at your macros.
The most anti-inflammatory version of keto looks a lot like a Mediterranean diet that happens to be very low in carbohydrates. If you’re building it around olive oil, fatty fish, and real vegetables, you’re probably on solid ground. If you’re building it around deli counters and cheese wax, don’t be surprised when your hs-CRP doesn’t budge.
