Most people find exogenous ketones after a rough week. They’ve been strict about carbs, they’re waiting for the fog to lift, and someone in a Facebook group swears that a $60 tub of BHB salts fixed everything. You might be wondering if it’s actually that simple, or if you’re about to spend money on something that sounds too good to be true.

Here’s the short answer: exogenous ketones are real, they work in a narrow technical sense, and they’re wildly oversold. Let me explain what I mean by both halves of that.

Your liver makes ketones when carbohydrate intake drops low enough and fat is being oxidized. That’s endogenous ketone production. Exogenous ketones are ketones you consume from outside the body, and they raise blood ketone levels regardless of what you’ve been eating. You don’t have to be in any particular metabolic state. Drink a serving of beta-hydroxybutyrate (BHB) salts and your blood will show elevated ketones within about 30 to 60 minutes.

That’s genuinely interesting physiology. The question is what it actually gets you.

The two forms you’ll encounter most often are BHB salts and ketone esters. BHB salts are BHB bound to a mineral, usually sodium, potassium, calcium, or magnesium. They’re the ones in most consumer products, including brands like Perfect Keto, Pruvit’s Keto//OS, and HVMN’s older formulations. Ketone esters are a different beast: they raise ketones higher, faster, and they taste genuinely awful. A few grams of ketone ester can push blood BHB above 3 mmol/L where typical salts might get you to 0.5 to 1.0 mmol/L. Esters are used in some research settings and endurance sports contexts but they’re expensive (sometimes $30 to $40 per serving) and not what most people are buying.

MCT oil is sometimes grouped here, but it’s not technically an exogenous ketone. It raises ketones indirectly by giving your liver the substrate to make them. Worth knowing the distinction. (If you’re interested in MCT oil, look for C8-dominant products like Bulletproof Brain Octane or Sports Research MCT Oil, which tend to produce a faster ketone response than blended MCT oils.)

What the Research Actually Shows

The honest summary: most of the solid research on exogenous ketones involves either animals or very specific human performance contexts, not the “lose weight faster and feel amazing” use case that’s marketed to most buyers.

Where there’s legitimate evidence: a 2022 study published in Cell Metabolism and several sports science trials have shown that ketone esters can meaningfully improve endurance performance in well-trained athletes, partly by sparing glycogen and partly through effects on muscle protein metabolism. That’s real. Researchers at the University of Oxford and groups studying Tour de France-level cyclists have been investigating this for years, and some elite teams have used ketone esters in competition.

For cognitive function, the evidence is suggestive but not settled. There are reasonable mechanistic reasons to think ketones might help in situations of acute cognitive demand, and some preliminary work in populations with Alzheimer’s risk or mild cognitive impairment looks interesting. But “might help people with neurodegenerative risk” is a different claim than “gives you mental clarity before your 9 a.m. meeting.”

Here’s where I’ll push back on something a lot of keto writers won’t say directly: exogenous ketones do not cause fat loss, and there’s a real argument that in some contexts they could slow it. Your body prioritizes burning the fuel it has available. If you flood your bloodstream with ketones from a supplement, you may actually reduce how much body fat you’re oxidizing in that window. The research on this is still developing, but the mechanism is sound enough that I’d be skeptical of any product marketed as a weight loss tool.

Where They Might Actually Be Useful

I’m not writing these products off entirely. There are situations where I think they’re worth trying.

The clearest one is managing the transition into ketosis, especially the first few days when fatigue, headaches, and brain fog hit hardest. Exogenous ketones won’t get you into ketosis faster, but they can raise blood BHB enough to blunt some of the symptoms while your body catches up. Think of it as a bridge, not a shortcut. This is probably the best-supported practical use for most people, and it’s the one I actually mention to clients.

The other reasonable use case is athletic performance, specifically endurance efforts where you want to test whether elevated ketones affect your output. If you’re doing long-distance cycling, trail running, or anything lasting 90 minutes or more, it might be worth experimenting, especially if you have access to a blood ketone meter (the Keto-Mojo GK+ is what I recommend; it’s accurate and the strips are reasonably priced compared to Abbott Precision strips).

For fasted workouts or intermittent fasting, some people find that a small dose of BHB salts helps them train without feeling depleted. That’s largely anecdotal, but the potential downside is low.

What I’d skip: using them as a daily supplement with no specific purpose, buying them to “stay in ketosis” while eating more carbs (it doesn’t work that way), and any product making weight loss claims.

The supplement space here is messy. A lot of BHB salt products contain more sodium than the label emphasizes, which matters if you’re already getting plenty from food. Some products use cheap blends where the BHB content per serving is lower than it looks. And the GI distress from higher doses of BHB salts is real. I’ve had clients work up to full servings gradually and that helps, but some people just don’t tolerate them well.

If you go in with clear expectations, they might be worth keeping around. If you’re hoping they’ll replace the hard work of actually adapting to a low-carb diet, they’ll disappoint you.
