You’ve been doing keto for three weeks, you’re staying under 20 grams of carbs, and you decide to reward yourself with a “keto brownie” made with a popular sweetener blend you grabbed at the grocery store. Two hours later, you’re stalled, bloated, and reading ingredient labels with a flashlight. The sweetener was the problem, and the bag never told you why.

Sweeteners are where keto gets complicated fast, and where a lot of otherwise solid low-carb eating quietly falls apart. The good news: there are genuinely good options. The bad news: there are more bad ones, and the marketing around them is aggressive enough to confuse even experienced low-carb eaters.

Here’s what actually matters.

Why Sweetener Choice Actually Matters on Keto

Not all zero-calorie sweeteners behave the same way in your body. Some raise blood glucose directly. Some trigger an insulin response without raising glucose. Some cause enough GI distress to ruin your afternoon. And some work beautifully for most people most of the time.

Here’s the thing that makes this matter on keto specifically: you’re working with a narrow carbohydrate window, usually 20 to 50 grams per day. A sweetener that causes even a modest insulin spike can interrupt ketosis for hours. Add in the fact that “net carb” calculations on packaged keto products are often misleading, and you’ve got a real problem hiding in plain sight.

There’s also the individual variation issue. I’ve seen clients tolerate erythritol with zero issues while their spouse can’t touch it without bloating. One person’s perfect sweetener is another person’s afternoon regret. That’s not a cop-out. It’s biology. And it’s why understanding the full landscape matters more than just picking “the best one” off a list.

Allulose is the best sweetener most people haven’t tried yet. It’s a rare sugar that occurs naturally in small amounts in figs and wheat. Your body absorbs it but doesn’t metabolize it for energy, so it contributes negligible calories and has essentially no effect on blood glucose or insulin in most people. It behaves like real sugar in cooking: it browns, it caramelizes, it dissolves cleanly. No cooling aftertaste. No digestive drama at normal doses.

The catch: it’s more expensive than most alternatives, and it’s not universally available in all countries (the EU still classifies it differently). In the US, the FDA ruled in 2019 that allulose doesn’t need to be counted in total or added sugars on nutrition labels, which tells you something about how it behaves metabolically.

Monk fruit extract (also called luo han guo) is the other tier-one option. It’s 150 to 250 times sweeter than sugar, so you use very little. It doesn’t raise blood glucose, doesn’t cause digestive issues, and has a clean sweetness with minimal aftertaste at moderate amounts. The problem is that pure monk fruit is expensive and hard to find. Most products labeled “monk fruit sweetener” are actually monk fruit blended with erythritol or allulose, which isn’t necessarily bad, but it means you’re not just buying monk fruit.

I keep both on my shelf. For baking, allulose is my go-to. For coffee or a quick smoothie, a few drops of liquid monk fruit extract does the job without the bulk.

The Tier-Two Sweeteners: Good but With Caveats

Erythritol is probably the most widely used keto sweetener, and for most people it works well. It’s a sugar alcohol that’s about 70% as sweet as sugar, has essentially zero glycemic impact, and contributes roughly 0.2 calories per gram (effectively negligible). It doesn’t metabolize the way other sugar alcohols do, so it doesn’t feed gut bacteria and cause the fermentation-related bloating that maltitol and sorbitol cause.

That said, erythritol has a noticeable cooling sensation, especially in unbaked or no-heat applications like frosting or mousse. Some people don’t mind it. Others find it genuinely unpleasant. And a small subset of people get digestive issues even with erythritol, particularly at higher amounts.

There’s also been a 2023 study from the Cleveland Clinic suggesting a possible association between high erythritol blood levels and cardiovascular events. The research is preliminary, the methodology involved feeding people large boluses of erythritol rather than typical dietary amounts, and the scientific community hasn’t reached consensus. Still, it’s worth knowing about. If you’re eating erythritol-heavy products daily in large quantities, it’s a reasonable thing to discuss with your doctor.

Stevia is familiar, widely available, and genuinely zero-glycemic. The problem is the taste. Pure stevia has a distinct bitterness and licorice-adjacent aftertaste that many people find difficult to work around, especially in larger amounts or baked goods. Liquid stevia in small doses, say, two drops in coffee, works fine. Trying to bake a cheesecake sweetened primarily with stevia is an exercise in frustration.

Stevia blends (stevia plus erythritol, or stevia plus inulin) often taste better than pure stevia alone, but read the label carefully because inulin, while beneficial as a prebiotic, can cause significant gas and bloating in sensitive people.

The Sweeteners to Avoid (or Use Very Carefully)

Maltitol is the biggest offender in packaged “sugar-free” and “keto-friendly” products. It has a glycemic index of around 35, which is lower than sugar’s 65, but still absolutely high enough to spike blood glucose and disrupt ketosis in most people. It’s cheap, which is why manufacturers love it. If you see maltitol in the first few ingredients of a keto snack, put it back.

Sucralose (Splenda) is complicated. The liquid form is fine for most people in small amounts. The granulated form, however, is typically bulked with maltodextrin, which has a glycemic index of 85 to 105, higher than table sugar. That’s not a typo. Check whether your sucralose product uses maltodextrin as filler.

Beyond the bulking agent issue, there’s ongoing debate about whether sucralose affects gut microbiome composition and whether it triggers cephalic phase insulin responses (a small insulin release triggered by sweetness perception alone). The evidence is mixed and often comes from studies using amounts much higher than typical consumption. I’d call sucralose acceptable in liquid form, occasionally, not ideal as a daily staple.

Xylitol is worth a brief mention: decent glycemic profile, acceptable taste, reasonably good for baking. But it causes significant GI distress in many people at amounts over 20 to 30 grams, and it’s severely toxic to dogs. The latter isn’t a nutritional concern for you personally, but if you have a dog and bake with xylitol, the risk is serious enough to mention plainly.

Swapping sweeteners isn’t always a one-to-one trade. Here’s how to approach it practically.

Step 1: Identify the role sugar plays in your recipe. Is it just sweetness? Or does the recipe also need browning, bulk, moisture retention, or the crystallization properties of sugar? This determines which substitutes will work.

Step 2: Match function, not just sweetness level. Allulose behaves closest to sugar functionally. Use it anywhere you want browning or caramelization: cookies, brownies, sauces, glazes. Erythritol provides bulk and sweetness but won’t brown the same way; it also re-crystallizes when cooled, which can make baked goods gritty.

Step 3: Use blends for complex baked goods. A mix of allulose and erythritol (roughly 50/50 by weight) gives you the functional browning of allulose with the cost-effectiveness and stability of erythritol. Many commercial keto baking blends use exactly this logic.

Step 4: Add liquid sweeteners at the end. Monk fruit extract and liquid stevia don’t contribute bulk. Add them after you’ve established your base measurements with a solid sweetener. Start with less than you think you need; both can turn bitter if you overshoot.

Step 5: Test your personal tolerance separately. Before you make a full batch of keto fudge with a new sweetener, eat a small amount of that sweetener on its own and wait a few hours. Digestive sensitivity is real and individual. Finding out during a party dessert is suboptimal.
